How to Apply
In order to start the process, you must request a screening for the EDCD Waiver from your local Department of Social Services office. The following is a list of contacts by city/county:

When you are directed to the appropriate person, request a screening for the EDCD Waiver. The person will ask you a few preliminary questions over the phone. When satisfied, the person will then schedule a screening with the individual seeking services and the individual’s parent/guardian if necessary.
The screening will take place in the individual’s home. The pre-admission screening team consists of a social worker from your local Department of Social Services and a nurse from your local Department of Health. A physician will also review your case, but is not present during the screening.
The screening team will be looking at many factors when weighing the risk of nursing facility placement such as the extent and type of the individual’s dependencies, the coping ability and resources of the individual’s support system, other demands already placed on the individual’s support system, and other available community resources.
The screening team will complete the Uniform Assessment Instrument (UAI) as they ask you questions and complete their assessment.
